At Priory Medical Group we strive for a positive safety culture and encourage all staff to be open and honest when things go wrong or not as expected. Recording incidents can proactively improve patient and staff safety and enhance the service we provide for our patients.
Once an incident is reported and investigated, we would encourage the whole team to be involved in a supportive discussion to allow reflection and learning from the event to improve patient care.
Remember incidents can be both positive and negative and we hope for an open and honest culture, with no blame, just understanding what we can do differently and how we can change to prevent similar events in the future.

Definitions
Incident | An incident can be anything that has not gone as well as expected, this could be a major event to a minor inconvenience. At PMG we use a single reporting form to capture in a timely fashion the event that has taken place. These incidents may be clinical, non-clinical or HR related, examples include a prescription error, a break in the cold chain or an abusive patient. |
Significant Event | A significant event is any unintended or unexpected incident, which could or did lead to harm of one or more patients. This includes incidents where the event should have been prevented (GMC). |
Learning Event | The incident has a poor outcome, but does not reach the GMC threshold for harm, it presents an opportunity for learning. |
