We are been asked to sign an increasing number of City of York Council Council Tax exemption forms. It is important patients are aware that only patients with severe and permanent mental illness or memory impairment are eligible for exemption. When we are asked to complete the form, we will either agree with the statement that the patient is eligible, or we can sign to say we do not agree following a review of the medical records.
